Nurse QA Specialist
The primary function of the Nursing QA Specialist is to review and audit Home Health Agency visit notes for documentation consistency with the patient’s Plan of Care and adherence to the quality of care guidelines set by Medicare in the United States.
What it takes:
- Bachelor’s degree in Nursing with active PRC license
- At least 1 year clinical experience, preferably in the adult medical-surgical area
- Skilled in providing effective nursing care, assessing patient situations
- Experience in a Healthcare BPO is A PLUS, but not required
- Willing to undergo onsite training (3 months onsite) in nursing QA review and Medicare guidelines
- Preferably with government IDs/numbers (SSS, Philhealth,Pag-Ibig & TIN) and valid NBI clearance
- Preferably with stable internet connection at home (for post training/WFH)
Duties and Responsibilities:
- Review all visit notes and clinical records for clients reimbursement
- Reference OASIS assessment and patient’s therapy evaluation for accuracy and completeness of each visit notes
- Audit charts for clinical oversight, compliance, and adherence to the US Medicare guidelines
- Identify clinical problem areas and document deficiency trends
- Submit daily QA work log report
PT QA Specialist
The primary function of the PT QA Specialist is to review and audit Home Health Agency therapy visit notes for documentation consistency with the patient’s therapy plan of care and adherence to the quality of care guidelines set by Medicare in the United States.
What it takes:
- Bachelor’s degree in Physical Therapy with active PRC license
- At least 1 year experience as a Physical Therapist (preferably in gerontology and home care)
- Working knowledge of rehab practices and procedures
- Experience in a Healthcare BPO is A PLUS, but not required
- Willing to be trained for PT QA review and Medicare guidelines
- Preferably with government IDs/numbers (SSS, Philhealth,Pag-Ibig & TIN) and valid NBI clearance
- Willing to work onsite (Robinsons Cybergate site)
Duties and Responsibilities:
- Review all visit notes and clinical records for clients reimbursement
- Reference OASIS assessment and patient’s therapy evaluation for accuracy and completeness of each visit notes
- Audit charts for clinical oversight, compliance and adherence to the US Medicare guidelines
- Identify clinical problem areas and document deficiency trends
- Communicate to clients administrative and clinical associate staff in the correction of these deficiencies
- Manage and submit daily QA work log report
Medical Coder
The primary function of the Medical Coder is to review medical charts and assign appropriate ICD coding and sequencing for Home Health Agency clients.
What it takes:
- Bachelor’s degree in Nursing or Physical Therapy with active PRC license a plus
- Proven experience in ICD9/10 coding & CPT code
- Must be analytically inclined and detailed-oriented
- AAPC Member (Certified Professional Coder) is a plus
Duties and Responsibilities:
- Review OASIS assessments and analyzes patient medical records to accurately assign ICD-10 CM codes for each diagnosis and procedures
- Ensure appropriate ICD-10 coding and sequencing as it relates to the patient’s medical condition including comorbidities
- Collaborate with client’s clinical associate staff to clarify any data integrity issues and/or correction of documentations
- Onboarding of Coding protocols for Qavalo clients
- Update self on the current trends on medical coding
- Meet daily productivity and quality goals set by the management
- Manage and submit daily QA work log report
- Other job duties deemed necessary
EMR Charting Specialist
The primary function of the EMR Charting Specialist is to convert the clinician’s written patient assessment into an electronic medical record by accurately encoding the patient assessment after the information has been verified and coincided with the patients referral document and other supporting medical documentations.
What it takes:
- Bachelor’s degree in Nursing with active PRC license
- At least 1 year clinical experience, preferably in the adult medical-surgical area
- Skilled in providing effective nursing care, assessing patient situations
- Experience in a Healthcare BPO is a plus, but not required
Duties and Responsibilities:
- Efficiently encodes clinicians patient assessment into the plotted daily visit notes in the EMR. These assessment forms include comprehensive assessments such as the different types of OASIS, therapy evaluations and the required skilled discipline daily visit notes.
- Organizes, compiles, sort out and process the clinical data to ensure a cohesive patient assessment record
- Coordinates with clinicians for verification or clarification of information that are illegible or does not match with the referral notes
- Ensures accuracy of the data entry by reconciling the clinical assessment with the MD referral notes and other supporting medical documentation
- Adheres to the agency data encoding protocols
- Maintains confidentiality in handling the patient’s information
- Follows the correct usage of data encoding tools
- Updates Data Encoding Tracker Sheet
- Complies with required daily productivity for data encoding
- Other job duties deemed necessary
